印度团签所需资料

客人身份证签发地为：广东，广西，湖南，海南，云南，福建，四川所提供资料如下

• 中文资料表格（由我公司提供，必须填写完整）
• 有效期半年以上的护照原件，尾页签名，连续3也空白签证页
• 彩色正面照片【2张】（照片：5cm*5cm.,美国签证照片尺寸，白色背景照片，头顶最少留0.5CM 空白）
• 身份证正反面的复印件【1份】，

备注：

• 18岁以下的客人提供校卡复印件和户口本复印件，如果客人和同行父母不在同一户口，请提供出生证。如果小孩跟父母一方去的话，要提供不去的那一方的护照首页或者身份证复印件。
• 香港和澳门护照，需要提供回乡证复印件。资料表必须填写英文地址。
• 身份证签发地为7省以外的，需提供在职证明。模板见下。蓝色字体，请根据实际情况更改。
